                        <p>LexisNexis<sup>®</sup> Risk Solutions provides information solutions to businesses and government that help them identify and manage risk. Our customers use our products to build a better picture of who they do business with, prevent and investigate fraud, investigate crimes and keep communities safer. We do business with banks and financial institutions, insurance carriers, healthcare providers and government agencies, law enforcement agencies, and non-profits.

                        <p>For example, companies use our tools to verify identities in order to offer loans, quote insurance rates, and offer government assisted benefits. The information we provide about consumers has been collected from public record and publicly-available data sources. We also use proprietary data and information from third-party data sources in compiling our information products. Consumers benefit when companies use our tools to make it harder for fraudsters to steal identities to get credit, loans or benefits they aren’t eligible for. If you would like to see the information that is maintained about you in our system, you may request a copy of your LexisNexis<sup>®</sup> Consumer Disclosure Report.</p>

                        <h3>Request and Receive a Report Online</h3>
                        <p>You can request and <a href="/consumer">receive reports from LexisNexis Risk Solutions online</a>. After your request is submitted, you will receive a letter via U.S. Mail with details explaining how to access your report online.</p>

                        <p>If you prefer to mail in your requests, you can choose to download a <a href="/consumer">Printable Request Form (PDF)</a> and send it to us via U.S. Mail. If it is more convenient to call us, feel free to <a href="/consumer">give us a call</a> and we will submit your requests for you.</p>

                        <h3>Exercise Your State Privacy Rights</h3>

                        <p>Certain states grant their residents options with respect to their personal information.                                 <h3>If you received an Adverse Action Letter</h3></a>
                                <p>An adverse action letter is sent when a carrier has not given a consumer the best rate for insurance or taken any of the following actions:</p>
                                <ul>
                                    <li>Denied you insurance</li>
                                    <li>Increased your rates</li>
                                    <li>Limited your coverage</li>
                                    <li>Cancelled your policy</li>
                                </ul>
                                <p>If your insurance company has sent you an adverse action letter, please contact the LexisNexis Consumer Center at <strong>1-800-456-6004</strong> to request the information related to the adverse action. Please be ready to provide your first and last name, Social Security Number, driver’s license number and state in which it was issued, date of birth, current home address and phone number. If your insurance carrier provided a reference number on the adverse action letter you received, please be ready to provide it, as well. This will help expedite the process.</p>
