Teletrack, you are entitled to receive a copy of
that same information free of charge. Consumers can
request a free copy of their report annually.
For a copy of your report, please mail your request
to Teletrack PO Box 740008 Atlanta, GA 30374.
Please include a copy of your proof of identity
and a document that confirms your address .
In addition, we will reinvestigate any information
that we maintain which you believe is inaccurate and
will correct any such information that is determined
to be inaccurate or incomplete.

<p class="p-body fs-6">The Fair Credit Reporting Act (the “FCRA”) allows Teletrack to include your name and address on prescreen lists used by creditors or insurers for sending you unsolicited offers of credit or insurance. If you would like your name and address to be removed from such prescreen lists for 5 years—”beginning 5 business days after the date Teletrack receives your request”—please call us at: (877) 309-5226.</p>
<p class="p-body fs-6">If you are unable to print the form, you may mail us your written request and include the following information:</p>
<ul class="p-body fs-6">
<li>Your first, middle and last name (include Jr., Sr., III, etc.) </li>
<li>Your current address</li>
<li>Your previous address (if you have moved in the last 6 months) </li>
<li>Your Social Security number and your date of birth</li>
<li>A statement whereby you request the permanent removal of your name from the prescreen lists that Teletrack provides to businesses that send firm offers of credit or insurance </li>
<li>Your signature</li>
</ul>
<p class="p-body fs-6">Please note the following:</p>
<p class="p-body fs-6">Removing your name from prescreen lists does not affect your ability to apply for or obtain credit or insurance.</p>
<p class="p-body fs-6">While your name will be permanently removed from the prescreen lists that Teletrack provides to businesses for the purpose of making a firm offer of credit or insurance, you may continue to receive offers from sources that do not use Teletrack to compile their prescreen lists.</p>
